Part One
The student must complete before the committee meeting.
Note: The student will receive an email with the responses. Please print this email as a PDF to be compiled with Part 2 and to provide to the Committee before the Meeting. If there are changes to the course of study that come up during the Meeting, the student should edit the Student Component of the Form before final submission.
Part Two
The committee must then complete the committee components of the Annual Graduate Committee Progress Assessment form. The committee must sign the last page and provide the PDF to the student to sign and compile.
Part Three
The student must combine part one and the signed Annual Graduate Committee Progress Assessment form into one PDF.
Please note the following:
- If the student is submitting their thesis/dissertation plan, that document should be added to the end of this PDF.
- PDFs can be combined for free online through .
- The final file should be named as follows: Student鈥檚 Last Name, First Name, underscore, 鈥淎nnualMeetingForm,鈥 underscore, and the year (example: DoeJane_AnnualMeetingForm_2020.)
- The single PDF with all the components must be emailed to lifesciences.gradcoord@unlv.edu by 11:59:59 p.m. on October 31.
